Significa que no la persona no es ya ni cerca de ser tan excelente como la otra persona.
Se puede usar con clases sociales
Ejemplo: La Niña rica le dijo tú no me llegas ni a los tobillos.

@serendipity17 Supone despreciar a esa persona y es ofensivo si se lo dices a la cara, directamente. Lo normal es que se diga respecto de alguien que no está y añadiendo “altura”, de esta forma.
Su novio (de ella) no le llega (a ella) ni a la altura de los tobillos.
Así querrías destacar que es una persona de inferior valía que la persona con quien le comparas. No tanto valía social, sino valía personal (menos inteligente, o menos generosa, o menos culta, o menos bondadosa,...) por contraste con las cualidades, en este caso, de la novia
