Abstract
The histopathologic diagnosis of panniculitis requires both careful morphologic assessment and close clinical correlation. The pathologist encountering a biopsy for panniculitis may narrow a differential diagnosis by utilizing an algorithmic approach, typified by classification as either a mostly septal versus mostly lobular pattern, assessment for the presence or absence of vasculitis, and characterization of predominant inflammatory cell types. In addition to these general patterns, there are some features, such as the hyalinized fat necrosis of lupus panniculitis, which enable a specific diagnosis to be made. As described in this chapter, consideration of these histopathologic details, paired with close clinical correlation, allows for both specific diagnoses of panniculitis and reasonable differential diagnoses in those cases defying simple classification.
