Presentation
Front seat passenger with facial trauma, presented to the emergency department with nasal bleeding.
Patient Data
fracture anterior nasal spine, intact morphology of the remaining facial bones
mild right-sided deviated nasal septum
partially opacified bilateral anterior ethmoid air cells and mild mucosal thickening of the left maxillary sinus; the remaining paranasal sinuses are well-aerated and clear, and bilateral mastoid air cells are also well-aerated and clear
multiple small sub-centimeter lymph nodes in the scanned upper neck bilaterally, likely reactive in nature
Reformatted axial and sagittal maximum intensity projection (MIP) images showing anterior nasal spine fracture (encircled).
Case Discussion
The anterior nasal spine (ANS) is a small midline bony projection of the maxilla at the level of the nares. Fracture of ANS is an uncommon nasal fracture which is overlooked by the patients and physicians and rarely documented in the literature, likely due to its small size and central location1.
