If your Medicare Card is lost, or your information needs to be changed, use the information below to request a new one.
I lost my Medicare Card. How do I get a new one?
Contact the Social Security Administration at 1-800-772-1213 to ask for a new card or visit the Social Security website to request a new card online.
You can also print a copy of your Medicare card from your medicare.gov account.
I recently moved. How do I change my address with Social Security and Medicare?
Call the Social Security Administration at 1-800-772-1213 to make your address change. You can also visit the Social Security Administration website at www.ssa.gov. If you are a Railroad Retiree, call 1-800-808-0772.
