Symbyax (olanzapine / fluoxetine) is a combination medication used to treat bipolar disorder, as well as depression when other depression medications haven't worked. It's the only combination medication that contains an antipsychotic and SSRI.

Symbyax (olanzapine / fluoxetine) is a combination medication made of two drugs that affect mood:
Olanzapine is an atypical antipsychotic medication that affects dopamine and serotonin levels in your brain, which helps control your thoughts, mood, and behaviors.
Fluoxetine is a selective serotonin reuptake inhibitor (SSRI) antidepressant. It raises serotonin activity in the brain, which helps regulate mood.

Along with its needed effects, a medicine may cause some unwanted effects. Although not all of these side effects may occur, if they do occur they may need medical attention.

Some side effects may occur that usually do not need medical attention. These side effects may go away during treatment as your body adjusts to the medicine. Also, your health care professional may be able to tell you about ways to prevent or reduce some of these side effects. Check with your health care professional if any of the following side effects continue or are bothersome or if you have any questions about them:

If you're currently taking olanzapine (Zyprexa, Zyprexa Zydis) and fluoxetine (Prozac) as separate medications, stop taking them before starting Symbyax (olanzapine / fluoxetine). You don't want to end up taking too much of the medications.
It's recommended to take Symbyax (olanzapine / fluoxetine) in the evening time, since sleepiness is a common side effect.
Make sure you drink enough fluids to stay hydrated. Symbyax (olanzapine / fluoxetine) can make it harder for your body to control its own temperature. Staying hydrated can prevent overheating.
Symbyax (olanzapine / fluoxetine) can cause a drop in blood pressure when changing positions, such as from sitting to standing. This is more common when you first start taking Symbyax (olanzapine / fluoxetine) or if your dose was recently changed. Be careful changing positions to prevent yourself from falling and hurting yourself.
Don't stop taking Symbyax (olanzapine / fluoxetine) without talking to you rprovider. Stopping this medication suddenly can cause symptoms, such as stomach issues and anxiety.
Symbyax (olanzapine / fluoxetine) can cause some serious health issues. This risk may be even higher for certain groups. If this worries you, talk to your doctor or pharmacist about other options.
Risk factors: Age 24 or younger
Symbyax (olanzapine / fluoxetine) can raise the risk of suicidal thoughts or behavior. Watch for new or worsening depression, suicidal thoughts or behavior, especially during the first few months of treatment or when the dose changes. Let your healthcare provider know immediately if you notice any unusual changes in mood or behavior while taking this medication.
Risk factors: Older adults with dementia-related psychosis
People over the age of 65 who also have dementia can sometimes have some of the symptoms of schizophrenia, like hallucinations. This is called dementia-related psychosis, and it should not be treated with Symbyax (olanzapine / fluoxetine). If Symbyax (olanzapine / fluoxetine) is used for dementia-related psychosis, it can cause strokes or even death. Please let your provider know if you or your loved one has dementia before starting Symbyax (olanzapine / fluoxetine).
Symbyax (olanzapine / fluoxetine) can cause a rare but life-threatening condition called neuroleptic malignant syndrome (NMS). Symptoms of NMS include high fever, muscle stiffness, confusion, sweating, and changes in heart rate and blood pressure. If you notice any of these symptoms, seek medical help immediately.
Symbyax (olanzapine / fluoxetine) can cause a rare but serious skin reaction called Drug Reaction with Eosinophilia (DRESS). Symptoms of DRESS include rash, fever, and swollen lymph nodes. If you develop any of these symptoms while taking Symbyax (olanzapine / fluoxetine) you should seek medical care.
Risk factors: History of diabetes or high cholesterol
Symbyax (olanzapine / fluoxetine) can raise your blood sugar, cholesterol levels, and make you have a larger appetite, all of which can lead to weight gain. In extreme cases, the rise in blood sugar can cause serious complications such as coma and death. If you experience any symptoms of high blood sugar like more thirst, hunger, or more frequent urination, let your provider know. Your provider might do blood tests regularly to make sure that your blood sugar and cholesterol are not too high.
Serotonin syndrome is a rare, but potentially life-threatening condition where there's too much serotonin active in the brain. Serotonin is one of the chemicals in your brain responsible for your mood. Your risk is higher if you take Symbyax (olanzapine / fluoxetine) with other medications that affect serotonin levels (e.g., antidepressants, lithium, triptans, St. John's wort). Symptoms of too much serotonin include fast heart rate, sweating, muscle stiffness or spasms, fever, and confusion. Tell your healthcare provider or seek medical help right away if you have any of these symptoms.
Risk factors: History of glaucoma
Many antidepressants, including fluoxetine, can cause eye-related side effects. This can cause eye pain in people who have underlying conditions such as glaucoma. Let your provider know if you have a history of eye problems and if the medication starts causing vision changes or discomfort.
Risk factors: Long-term use | Taking high doses
Symbyax (olanzapine / fluoxetine) can cause uncontrollable movements involving your face, tongue, and other body parts, especially if you've been taking Symbyax (olanzapine / fluoxetine) for a long time, or have been taking high doses. Symptoms include shaking, muscle stiffness, or feeling like you can't sit still. Tell your provider right away if you notice any unusual movements or restlessness since it can be hard to get rid of.

Symbyax (olanzapine / fluoxetine) can cause a sudden drop in blood pressure, sleepiness, and problems with balance, especially when you first start the medication or after your dose is raised. Dizziness, falls, and fainting can happen due to a sudden drop in blood pressure when you change positions, such as going from sitting to standing. To avoid falling, change positions slowly until your body is used to the medication.
Risk factors: Older adults with Alzheimer's dementia
Symbyax (olanzapine / fluoxetine) can sometimes make it hard for you to swallow food or liquids. This can be dangerous if food or liquids get into your lungs, because it can cause issues with your breathing and infections. Make sure to chew your food completely and swallow carefully if you notice tightening of your throat or neck area.
Risk factors: History of low white blood cell count
Symbyax (olanzapine / fluoxetine) can lower the amount of white blood cells in your body. White blood cells help your body to fight off infections and diseases. If you notice a fever or that you're getting sick more easily, let your provider know right away. Blood tests might be done before you start Symbyax (olanzapine / fluoxetine) and during the first few months of being on this medication to make sure that you have a healthy amount of white blood cells.
Risk factors: History of seizures | Health conditions that make it easier to get seizures
Symbyax (olanzapine / fluoxetine) can make you more likely to have seizures. Before starting this medication, let your provider know if you have a history of seizures or have a medical condition that puts you at risk for getting seizures.
Risk factors: Taking other blood-thinning drugs
Symbyax (olanzapine / fluoxetine) can raise your risk of bleeding or bruising, especially if you're also taking aspirin, NSAIDs, or blood thinners. This can be life-threatening. Tell your healthcare provider about any bruising that doesn't improve, black-colored stools, pink or red urine, or unusual bleeding from the nose or gums. If you're on blood thinners, the provider might have to change your dose.
Risk factors: Older adults | Taking diuretic medications
Symbyax (olanzapine / fluoxetine) can cause very low sodium levels in your blood. Tell your healtcare provider right away if you experience weakness, confusion, difficulty concentrating, headache, or memory problems.

Symbyax (olanzapine / fluoxetine) can affect the natural rhythm of your heart and cause it to beat abnormally. If you experience symptoms such as fast heart rate, dizziness, difficulty breathing, or chest pain, go to the emergency room right away. If you've had heart problems in the past, talk to your healthcare provider first to make sure Symbyax (olanzapine / fluoxetine) is safe for you to take.
Symbyax (olanzapine / fluoxetine) can cause prolactin levels in the blood to go up. High prolactin levels can lead to fertility problems, enlarged male breasts, sexual dysfunction, absent or irregular menstrual periods, and weak bones. Your provider may do regular blood tests to make sure that prolactin levels are within normal range.
Adults: The typical starting dose for Symbyax is 6 mg/25 mg (olanzapine/fluoxetine) by mouth once a day in the evening.
Children (ages 10-17 years): The typical starting dose for Symbyax is 3 mg/25 mg (olanzapine/fluoxetine) by mouth once a day in the evening.
Using this medicine with any of the following medicines is not recommended. Your doctor may decide not to treat you with this medication or change some of the other medicines you take.
Using this medicine with any of the following medicines is usually not recommended, but may be required in some cases. If both medicines are prescribed together, your doctor may change the dose or how often you use one or both of the medicines.
Using this medicine with any of the following medicines may cause an increased risk of certain side effects, but using both drugs may be the best treatment for you. If both medicines are prescribed together, your doctor may change the dose or how often you use one or both of the medicines.
Using MAOIs within 5 weeks of stopping treatment with Symbyax (olanzapine / fluoxetine), or within 14 days of stopping an MAOI
Use with pimozide or thioridazine
