Co-production and co-creation are increasingly valued as an approach to managing, coordinating and improving healthcare. These approaches are intended to involve patients, service users and the public along with health and social care providers. Although they are increasingly being seen as a promising way of transforming public services, with potential to generate renewed energy and bold ideas, to date the evidence of their impact has been limited. Healthwatch organisations listen to users of health and social care services, take information about their needs and experiences and share it with those involved in service commissioning and delivery to help identify potential improvements. We studied five Healthwatch organisations, all working closely with wider health and care systems, and found that while they clearly created value and had a degree of impact, in many cases their contributions tended to be more conservative than bold.
