ASSESSMENT NURSING

INTERVENTIONS

RATIONALE EVALUATION

Subjective: “Simula nung napanganak ko si baby sobrang hina talaga ng lumalabas na milk sa’kin at pakiramdam ko kulang yung nakukuha ni baby” as verbalized by the patient (newborn’s mother).

Objective:  (-) engorgement  The infant was being fed using a dropper.

Ineffective breastfeeding related to insufficient breastmilk production.

Childbirth

Breastfeeding hormone prolactin and oxytocin kicks in when the placenta is delivered

Milk production starts

Poor sucking/inappropriate breastfeeding style and technique

After 6 hours of nursing interventions:

 Mother can adapt ways to increase her milk supply.

 Initiate/instruct mother to do proper latching and let the baby suck.

 Inform the mother to eat hot soupy foods with green leafy vegetables and fruits.

 Instruct mother to increase fluid intake.

 Demonstrate proper way to put warm compress and the breast massage technique to increase milk supply naturally.

 The hormone prolactin is responsible for milk production, and the more a baby suckles, the more milk is produced.

 Eating hot soupy foods with green leafy vegetables such as malunggay leaves and broccoli makes the blood vessels more flexible, which makes it easier for milk to flow.

 Since water makes up over 90% of breast milk, it's essential to drink plenty of water at least 2 liters or more to keep your milk supply high.

 You can get your milk flowing more quickly by using moist heat, which improves circulation, opens your milk ducts, and stimulates let down of breastmilk.

After 6 hours of nursing interventions, the goal was fully met.

 After 6 hours, there was increased milk production from the mother’s breast.

 The spouse and other caregivers of the mother prepares hot soupy meals with healthy leafy vegetables for the mother.

 The mother’s daily water intake increases to 2 liters a day and more

 Breast massaging techniques were learned by the mother and breastmilk production improved.

 Mother showed

 Suggest using a breast pump or hand expression.

 Continued breast stimulation cues the mother’s body that more milk is needed, increasing supply.
