Diseases caused by Hepatitis B virus (HBV) have a worldwide distribution. Pakistan adopted the recommendations of World Health Organization (WHO) for routine universal infant vaccination against hepatitis B in 2002, currently being administered at 6, 10, and 14 weeks of age in a combination vaccine. This study was conducted to determine the immune response & anamnestic immune response in children, 9 months-10 years of age, after a 3dose primary Hepatitis B vaccination. This cross sectional study was conducted in the Department of Paediatrics, King Edward Medical University/Mayo Hospital, Lahore, Pakistan, from January to June, 2014. Immune response in infants after universal hepatitis B vaccination: a community-based study in Malaysia2013 •
INTRODUCTION This study aimed to assess the immune response in infants who received the three-shot hepatitis B vaccine in Malaysia. METHODS Consecutive infants born between March 2002 and April 2010 who received three doses of hepatitis B vaccine at a community clinic in Malaysia were enrolled in the study. Screening for hepatitis B surface antigen (HBsAg) and antibody against HBsAg (anti-HBs) was performed after the completion of primary immunisation, at approximately one year of age. RESULTS A total of 572 infants (median age 9.3 +/- 2.7 months; range 6.3-48 months) were screened for immune response to hepatitis B vaccination - 553 (96.7%) infants had adequate levels of anti-HBs (>= 10 IU/L). Of the 440 mothers whose HBsAg status was known, 14 (3.2%) were positive for HBsAg. None of the 14 infants who were born to HBsAg-positive mothers were positive for HBsAg, and all but one infant had anti-HBs level >= 10 IU/L. Gender, gestational age and maternal HBsAg status were not found to significantly affect the subsequent immune response in infants following vaccination. CONCLUSION The proportion of Malaysian mothers who are positive for HBsAg remains high. The three-shot hepatitis B vaccine, given as part of universal vaccination against hepatitis B, provides adequate anti-HBs in the vast majority of infants in a community setting in Malaysia.

The study was undertaken, first, to determine the coverage rate of hepatitis B (HB) vaccine and second to evaluate the immune response to HB vaccine among children under 10 years old by measuring the level of circulating anti-HB surface antigen (anti-HBs) antibodies after immunisation with three doses. First, 840 children were randomly selected from 4 randomly selected sites in Sana'a city to study the coverage rate of the vaccine; of these, 504 children vaccinated against HBV prior to the study, were tested (56% males and 44% females). Sera were tested for anti-HBs antibodies by ELISA quantitative technique. Each individual's data was collected in a pre-designed questionnaire including: vaccination date, sex, and age at the time of the study. The coverage rate of HBV vaccine was only 69.9%, being slightly higher among male children (72.1%) than female children (66.8%). Background: Few data are available concerning the long term immunogenicity of the pediatric doses of hepatitis B vaccines. The aim: was to evaluate the immune response among school children after hepatitis B vaccination according to Expanded Program on Immunization (EPI) in Egypt, and to determine factors influencing the immune response to hepatitis B vaccine. Subjects and Methods: This cross-sectional study included 300 healthy school children who had been given three doses of hepatitis B vaccine during obligatory immunization schedule in Egypt. HBs antibody concentration was quantitated using Enzyme Linked ImmunoSorbent Assay (ELISA) and for seronegative cases (Anti-HBs Ab<10 mIU/ml), hepatitis B surface Antigen (HBs Ag)was done to exclude hepatitis B virus (HBV) infection. Results: Among 300 school children, 43.7% of them were seroprotective. HBs Ab titre decrease significantly with increasing age and with increase duration post vaccination as seroprotection rates (>10 mIU/ml)in studied subjects from 6-10 y, 10-13 y and 13-17 years old were 69.5%, 25.2% and 5.3% respectively, also significant decrease was found in males than females. There was no significant association between HBs Ab titre and other factors that affect the immune response to HB vaccine. HBs Ag was negative in 100% of studied seronegative children. Conclusion: Our results revealed that 43.7%of the studied children were HBV seroprotective. HBs Ab titre decrease significantly with increasing age, duration after vaccination and with male gender. Booster dose of hepatitis B vaccine should be considered to enhance the immune protection of the vaccine especially in our endemic area.
