About Early use of PPAM AID assists in the reduction of stump oedema and early mobility of the patient has obvious advantages, psychological and physiological. PPAM AID is suitable for Below Knee, Knee Disarticulation and Long Above Knee amputees. To suit the larger amputee, larger versions of the 650mm long, 750mm long, and 850mm long frames are now available. For all sizes, the girth of the top ring is increased by 10cm. Larger BK and AK Bags, and Cushion Bags are also available for use with these larger frames. Pending a satisfactory wound assessment by a Surgeon, the PPAM AID may be used from 5 days post-operative. With a new patient, the device should be applied for a 5 minute trial period at a pressure lower than 40mm Hg recommended for walking. The bag should then be removed and the stump carefully inspected for signs of adverse pressure, wound seepage, etc. Trial inflations should be continued, over several days if necessary, until a pressure of 40mm Hg can be tolerated. The stump should be checked prior to and following each session. We would recommend that the inflation time is slowly increased from 5 minutes to a maximum of 2 hours, twice daily. During periods of rest deflate the outer bag a little and elevate. The PPAM AID system is designed for partial weight bearing and on no account should this be used as a substitute for a prosthesis. Initially this device should only be used within parallel bars, progressing to crutches for the fitter patient. On no account should this be used with walking sticks alone. The stump needs to be covered as direct skin contact can result in excessive perspiration.

The appliance can be placed over soft dressings, bandaging or a plaster cast. If the patient is wearing trousers the excess length can be turned up and the device applied over the outside of the trouser leg. Care should be taken to avoid creasing. The use of a Shoulder Strap is highly recommended for suspension, especially for AK use. It should pass over the opposite shoulder. The PPAM AID pump will be checked and the gauge calibrated before leaving the factory and we recommend the gauge is checked for accuracy at regular intervals. Plus or minus 3mm Hg is considered acceptable. If you are concerned, please send the pump back to Ortho Europe for further testing.
Cleaning and storage instructions Clean bag using warm water a mild detergent. More stubborn stains may be removed using surgical or methylated spirit. Steam sterilising may be employed but only for a brief period, i.e. 2-3 minutes maximum. Allow to dry at room temperature. Leave deflated bags in the flat position during storage. PPAM AID is designed for use under professional supervision and is not for home use. Do not use in the presence of a wound infection. Not suitable for Bilateral Fitting.

Application on Below-Knee & Knee Disarticulation Stumps. 1.
The device can be applied over soft dressings and a stump bandage, or over a plaster cast. If the patient is wearing trousers the aid can be applied over the outside of the trouser leg after the excess length has been turned up.

A little air is pumped into the small, inner cushion bag. Close the valve.

The rounded end of the cushion bag is invaginated in on itself to form a shallow cup. This is placed over the end of the stump so as to cover not more than the distal 5cms. Do not attempt to insert the whole of the stump into the invaginated end of the cushion bag. Keep the tube away from the scar line.

The Below-Knee bag is selected. Holding the cushion bag in place, the outer bag is slid over the patient’s leg until the top is in contact with the patient’s groin.

A rigid frame of suitable length is selected and slid up over the outside of the outer bag until the sole of the rocker foot is in the desired position.

The ring at the upper end of the frame should lie at least 8cm below the top of the outer bag. It is advisable to keep the frame 2-3cm longer than the contralateral limb, as some shortening will occur on weight bearing.

The outer bag is inflated to a maximum 40mm Hg for walking. A pump with an accurate pressure gauge must be used. The frame should be supported during this procedure. The inflation pressure should be checked, as it may take a minute or two to stabilise. The crucible sling support is affixed to the frame so that it gives support to the distal end of the outer bag.

The patient can then stand within parallel bars (or if necessary remain seated) while the length is checked.

To adjust the length, deflate the main bag sufficiently to allow the frame to be repositioned and then reinflate the bag to 40mm Hg. There will be some shortening as the patient bears weight onto the device but this should not be more than 2-3cm.
The stump must be checked prior to application.
Application on Above-Knee Stump Follow the basic instructions for Below-Knee application with the following amendments: 1. As the lower end of the stump usually has a larger circumference than the orifice in the cushion bag, the bag is inflated slightly and folded in half. It is inserted into the outer bag to act as a cushion at the distal end of the stump. 2.
The above Above-Knee bag must be selected. This is distinguished by a collar ring at the proximal end, and its four vertical air pockets. It should be applied with the higher side to the lateral side of the stump.

The crucible sling support is optional as it may not be in appropriate position. The use of a shoulder strap is recommended to increase support, it should pass over the opposite shoulder.

ASSEMBLY AND FITTING
1.
A Quick Release Male Valve is needed to convert your existing PPAM bags in order for you to be able to use them with the new PPAM Aid hand pump or the new PPAM Aid foot pump.
2.
Hold the black tube securely at the shoulder joint and the valve housing at the other end and remove the black valve.
3.
Take the replacement valve and insert the ribbed end into the black tube.

ASSEMBLY AND FITTING 1. 2.
The valve kit consists of 1 female and 6 male quick release valves. A quick release valve kit is needed when you want to convert either your existing PPAM bags and/or existing PPAM foot pump.
Changing the valves on your old PPAM bags: 3.
Hold the black tube securely at the shoulder joint and the valve housing at the other end and remove the black valve.

Take the replacement valve and insert the ribbed end into the black tube.
Converting your old PPAM Aid foot pump: 5.
Take the quick release female valve and insert the ribbed end into the open end of the hose. In case the end of the hose is a bit loose (from using it with the old, black valves) use a knife to cut off a few centimetres to ensure the new female valve fits snugly after inserted. If it is too tight, put the end of the hose into boiling water for 30 seconds then insert valve.
